I have the NavTv unit in my '06 with factory nav and Bluetooth. I am using only the audio (fiber optic) part so the nav screen does not have to be up for it to work. I use the iPod for display and control. I personally don't like the screen up unless I am using the nav.
This is how it works: With the switch on (you need an on/off toogle switch installed) the audio in the car shuts down on any source and allowes an outside unit to play though the fiber optic system. If the switch is left on at all times, what I do, each time the key is cycled the factory radio source will play for 2 or 3 seconds, cut out, and then allow the outside souce (iPod) to play. If the switch is left off all radio/cd funtions work normally. If you have factory Bluetooth, as I do, if you recieve a call, audio from the ipod will mute, but you will need to turn the navtv unit off then back on to resume audio play.
I haven't had any problems with the unit and works as NavTv advertised. No issues as you describe. Hope this helps.

On the installation side the only thing I can think of that needs to be paid special attention to is how the fiber optic cables are looped and installed through the NavTv unit. The system won't work properly if the in/out fiber loop is not maintained. There are very specific instructions on what cables to use and how they are looped.
